Overview: Designed for sites ready to engage in deeper implementation and sustainability planning, this package offers intensive support across multiple dimensions of system change.

Engagement and Support: Extended engagement including in-depth training, ongoing consultation, and structured TA over the course of a year.

Key Deliverables:

  • Strategic curriculum and extended training series
  • A collaboratively developed Capstone Proposal (Implementation Plan) outlining system change goals, core strategies, and sustainability considerations
  • A detailed workplan specifying tasks, responsible parties, timeframe, and resource needs to operationalize the implementation plan
  • Milestone reviews to assess baseline needs, progress toward Capstone goals, identify challenges, and refine implementation strategies
  • Optional stakeholder engagement sessions to broaden buy-in
  • Induction into CYJ’s Fellows Network upon Program completion
